To find out which plans you and your dependents are enrolled in, review your benefit elections through the ITC Portal > Employee Self Service > View Your Benefit Elections Report.
The summaries below describe the types of wellness/preventive treatments are eligible for the wellness incentives for each plan.
- Accidental Injury Wellness Incentive Summary and AI Wellness Claim Form
(Accidental Injury Program Incentives: Low Plan: $50; Medium Plan: $75; High Plan: $100) - Critical Illness Wellness Incentive Summary and CI Wellness Claim Form
(Critical Illness Program Incentive: $100) - Cancer Plans Wellness Incentive Summary and Cancer Wellness Claim Form
(Cancer Plan Incentives: Low Plan: $50 to High Plan: $100)
The claim forms for AI and CI contain a website link where you can complete and submit an online claim form. The MetLife Cancer Plans wellness claims may be phoned in, emailed, faxed or mailed. You have up to 15 months from date of service to file AI, CI and MetLife Cancer Plan wellness claims.

- AbleTo Coaching. A personalized, coach-led virtual behavioral coaching program with dedicated 1-on-1 weekly coaching calls via phone or video chat plus in-app messaging between sessions for those managing stress, anxiety or depression. At No Cost.
- Teladoc. Tools and features to manage hypertension, diabetes prevention and diabetes management at no cost to you.
- Make sure you know which urgent cares and hospitals are in-network. To find out, use the UnitedHealthcare app, go to myuhc.com or call the number on your UHC member ID card.
- Health Advocacy (a New York Life Group Benefit Solutions program) finds options for non-covered and alternative health services or helps address questions and concerns related to your medical bills.
