Wellness Evaluations

As part of SwRI’s Health Improvement Program (HIP), registration for the 2025 Wellness Evaluation Program is now open and we welcome back Quest Diagnostics.

All regular employees covered under a SwRI health plan, or an external health plan, and all spouses covered under a SwRI health plan are eligible to participate at no cost. All participants must register to participate in the screenings. Walk-ins are unavailable. Participants have the option to choose between participating in an on-campus evaluation beginning in mid-July or making an appointment to visit their closest Quest Patient Service Center (PSC). Participant results are not shared with any individual or group at SwRI. All participants will receive an in-depth My Health Profile report for review and further follow-up with their personal physician. The Health Risk Assessment (HRA)/Questionnaire located within your Quest account portal must be completed to receive your results.

To register:

1. Visit my.questforhealth.com. If unable to register online, or have questions regarding the registration process, contact Quest at 1-855-623-9355.
2. New participants use SWRI2025 as the Registration Key, enter employee id number in the requested Unique ID (UID) field without the leading zeroes (e.g., 14111). Eligible spouses and domestic partners will use the same Registration Key and employee id number in the UID field but add an “S” at the end of the employee id number (e.g. 14111S).
3. Returning participants should reset their password and then log-in to the portal to complete registration.

Quest Diagnostics will send emails and other communications regarding the 2025 Wellness Evaluations from participant@email.questdiagnostics.com, From Name: Quest Diagnostics.

Slick Café: July 22, 23, 24; August 25, 26, 27; September 23, 24, 25; December 16

Fitness Center: July 28, 29; August 1; October 28, 29

Participants may also register for a wellness evaluation to be conducted at a Quest Diagnostics Patient Service Center (PSC) location until December 31, 2025.

All employees (see eligibility above) who participate will receive a $50 Amazon gift card following participation via email a few weeks following their screenings. Additionally, employees and spouses covered in the Critical Illness or Accidental Injury plan are also eligible for a health screening benefit of $50-$100 upon filing a claim with Cigna following the screening. Claims can be filed online at http://my.cigna.com or via the following:

• Accidental Injury Wellness Claim Form – Provides one annual benefit ($50–$100) for range of preventive services
• Critical Illness Wellness Claim Form – Benefit of $100 per covered person, per calendar year, for a health screening or diagnostic test

Colorectal Screening

Colorectal Screenings are back for 2025. This Colorectal Cancer screening kit is a voluntary offering and will be available by request for regular employees and spouses covered under an Institute health plan aged 45 and over. The Insure ONE collection kit will be mailed within two weeks from request and will contain what you need to complete your colorectal cancer screening in the privacy of your own home. This offering is voluntary and is separate from the blood work annually included in the biometric screenings.
